Managing Agent Authorization Form – Action Required for Applicable Shareholders

Dear Shareholders,

If you use a managing agent for your unit(s), please complete this Managing Agent Authorization Form.

Why this form is important

This form helps the Management Office:

• Work more efficiently with managing agents
• Maintain accurate records to support building security

What you need to provide

• Enter your managing agent’s details
• Define what the agent is allowed to do on your behalf

Submission guidelines

• If one agent manages multiple units with the same authorization, you may submit one form listing all units
• If you have different agents or different authorizations, please submit separate forms for each unit

Additional requirement

Please also submit:

• A clear, front-facing photo of the managing agent
• If the agent is a company, provide a photo of the primary contact person

Deadline

May 7, 2026

Important notice

Starting May 8, 2026, the Management Office must have a completed and notarized form on file.

• Send the form and photo to: office@riverplacenorth.org
• If no valid form is on file, we will communicate directly with the homeowner on all unit-related matters
